摘要
目的:探讨原发性肺癌标本中的层粘蛋白和Ⅳ型胶原表达的意义。方法:采用免疫组化LSAB法检测179例肺癌标本中层粘蛋白和Ⅳ型胶原的表达。结果:层粘蛋白及Ⅳ型胶原的表达强度在3组不同分级的肺鳞癌中差异具有极显著性意义(P<0.01),且中、高分化的肺鳞癌的层粘蛋白及Ⅳ型胶原的表达强度明显不同于低分化者(P<0.01),层粘蛋白和Ⅳ型胶原的表达强度在有无淋巴结转移两组间,差异亦具有显著性意义(P<0.05),层粘蛋白和Ⅳ型胶原的表达强度在3组不同术后生存期的肺鳞癌中差异亦具有显著性意义(P<0.05,0.01),5年以上生存组层粘蛋白的表达强度明显不同于半年内死亡者(P<0.01),半年内死亡组的Ⅳ型胶原的表达强度明显不同于其它两组(P<0.05,0.01),Ⅳ型胶原的表达强度在3组不同生存期的肺腺癌间,差异具有显著性意义(P<0.05)。结论:肺鳞癌的某些生物学行为不同于肺腺癌,层粘蛋白和Ⅳ型胶原的表达可作为评估肺鳞癌细胞的分化、淋巴结转移及预后的有价值的指标。
Purpose To study the relationship between the expression of laminin, collagen Ⅳ and the clinical pathology of the primary lung carcinoma. Methods The expressions of laminin and collagen Ⅳ in 179 cases lung carcinoma were detected with LSAB method. Results The expressions of laminin and collagen Ⅳ were significantly different among the three groups of lung squamous cell carcinoma with varying differentiation (P <0 01). They were different between grading Ⅰ,grading Ⅱ and the grading Ⅲ( P <0 01), so were the two groups of squamous cells carcinoma with lymph node positivity and negativity one ( P <0 05). The expressions of laminin and collagen Ⅳ were also significantly different among the three groups of squamous cells carcinoma with varying living time ( P <0 05,0 01),The expression of laminin was obviously different between the group died in half a year and the group surviving more than 5 years ( P <0 01), so was the expression of collagen Ⅳ between the group died in half a year and other two groups ( P <0 05,0 01). The expression of collagen Ⅳ was obviously different among the three groups of adenocarcinoma with varying living time (P <0 05).Conclusion The biological behavior of lung squamous cell carcinoma is somehow different from that of lung adenocarcinoma. The expression of laminin and collagen Ⅳ in squamous cell carcinoma is a valuable parameter on evaluating cell differentiation, metastasis and prognosis.
