颈椎前路减压植骨内固定治疗脊髓型颈椎病 被引量:4

Management of Cervical Spondylotic Myelopathy by Anterior Cervical Decompression,Autograft Bone Fusion and Plate Fixation

摘要 目的:探讨颈椎前路减压植骨内固定治疗脊髓型颈椎病的临床疗效。方法:对2001~2008年采用颈椎前路减压、取髂骨植骨融合、前路自锁钢板内固定治疗的61例脊髓型颈椎病病例进行回顾总结。结果:经8~24个月平均12.6个月随访,大部分患者症状明显缓解,部分患者症状基本消失,颈椎活动无明显受限。术后JOA评分平均改善率为85.2%,其中优31例,占50.8%;良21例,占34.4%;中8例,占13.1%;差1例,占1.6%。结论:颈椎前路减压植骨内固定治疗能直接解除脊髓受压、重建颈椎稳定性,是治疗脊髓型颈椎病的有效方法。 Objective:To evaluate the clinical efficiency of cervical spondylotic myelopathy treated by anterior cervical decompression,autograft bone fusion and plate fixation.Methods:The clinical results of 61 cases of cervical spondylotic myelopathy treated by anterior cervical decompression,autograft bone fusion and plate fixation from 2001 to 2008 were analyzed retrospectively.Results:During the average follow-up 12.6 months(8-24months),symptoms improved obviously in most patients and disappeared in partial patients,and the movement of cervical vertebrae had not obvious limit.According to postoperative JOA score,the ratio of improvement in neurological function was 85.2%,including excellent in 31 cases(50.8%),good in 21 cases(34.4%),fair in 8 case(13.1%) and bad in 1cases(1.6%).Conclusion:Anterior cervical decompression,autograft fusion and plate fixation can finish decompression to cervical cord safely and reconstruct stability of cervical vertebrae,is a rational and effective methods for the surgical treatment of cervical spondylotic myelopathy.
