吻合血管的阔筋膜瓣移植治疗慢性骨髓炎

Treatment of Chronic Osteomyelitis with the Fascia Latae Flap Transplantation with Microvascular Anastomoses

目的：报告应用吻合血管的阔筋膜瓣移植，联合游离植皮治疗慢性骨髓炎的临床结果。方法：治疗６例慢性骨髓炎伴软组织缺损。切取的阔筋膜瓣最大６ｃｍ×１６ｃｍ，最小６ｃｍ×７ｃｍ。结果：移植的阔筋膜瓣全部成活，取得了满意效果。结论：该筋膜瓣具有较薄、质地软、可修剪成各种形状以及血管蒂长等优点。是以旋股外侧动脉降支为其血供，血运丰富。

Objective:To report the clinical results of the treatment of chronic osteomyelitis with the fascia latae flap by microvascul aranastomoses and a combination of free skin graft.Method:Six cases of chronic osteomyelitis with soft tissue defect were treated.The sizes of the fascia latae flaps used ranged from 6 cm×7 cm～6 cm×16 cm.Result:All the fascia latae flaps survived completely and satisfactory results were obtained.Conclusion:The fascia flap has the advantages of being thin,soft,able to conform with various shapes,and has a long vascular pedicle.The fascia flap is nourished by the desending branch of the lateral circumflex femoral artery,highly vascularized and suitable for treatment of chronic osteomyelitis with soft tissue defect.
