结核性脑膜炎53例早期辅助检查分析

An Analysis of the Auxiliary Examination for 53 Cases of Tuberculous Meningitis

目的:探讨相关辅助检查诊疗结核性脑膜炎的价值。方法:回顾性分析2002年6月至2006年8月在昆明医学院第一附属医院神经内科住院治疗的53例结核性脑膜炎患者的临床资料。结果:脑脊液测压、细胞学及生化检查异常率高,是重要的实验室检查方法。头颅CT和MRI增强扫描异常率也高,有特征性表现,也应作为常规检查;胸片可以为结核性脑膜炎寻找佐证。结论:结核性脑膜炎临床存在较多不典型之处,及时、合理运用相关辅助检查对正确诊治结核性脑膜炎意义重大。

Objective： To investigate the feasibility of auxiliary examination for diagnosing the tuberculous meningitis （TBM）. Methods： Clinical samples of 53 cases with TBM were retrospectively analyzed in the department of neurology of the first affiliated hospital of Kunming Medical College from Jun, 2002 to Aug, 2006. Results： The comprehensive analysis shows that cytology and biochemistry of cerebrospinal fluid （CSF） and measurement of the initial cranial pressure have a high abnormal ratio and are a very important laboratory investigation. MRI / CT intensed scan has a high abnormal ratio and specificity also and should be routinely performed. X-ray may provide evidence for TBM. Conclusion： There is no investigation as a golden standard for TBM diagnose. To perform some available auxiliary laboratory investigations for diagnose early is very important to the patients with TBM.
