摘要
目的探讨膜型Ⅰ型金属蛋白酶(MT1-MMP/MMP-14)在宫颈鳞状细胞癌组织中的表达及临床意义。方法用免疫组化法检测60点(24例)宫颈鳞状细胞癌、癌旁及其边缘组织组合芯片、21例宫颈鳞癌组织、6例癌旁组织、15枚转移淋巴结组织中MMP-14蛋白的表达情况。收集相关病理学资料进行统计学分析,研究T分期、分化、浸润深度、淋巴结转移与MMP-14蛋白的相关性。结果MMP-14蛋白在肿瘤细胞的胞浆和胞膜中有强阳性表达。不同期别间的肿瘤细胞胞膜的MMP-14蛋白的强阳性表达率高于胞浆表达率(P<0.05)。有浸润的肿瘤的癌巢周边的间质细胞有MMP-14蛋白的弱表达。癌旁靠近基底膜的上皮细胞胞浆中有MMP-14蛋白的表达,强阳性表达率为12%。转移淋巴结的肿瘤细胞胞浆中有黄色颗粒,强阳性表达率为3%。正常宫颈组织不表达或弱阳性表达。原位灶与癌旁MMP-14蛋白的表达差异有统计学意义(P=0.000)。癌旁与边缘组织表达差异无统计学意义(P>0.25)。结论MMP-14蛋白主要在肿瘤细胞的胞浆及胞膜中表达,其高表达与宫颈鳞癌的淋巴结转移有关,在宫颈癌的浸润、转移中起重要作用,可能成为宫颈鳞癌淋巴结转移的预测因子。
Objective To investigate the expression of memberane type Ⅰ matric metalloproteinase(MMP-14) in squamous cell carcinomas of cervix and to explore their clinicopathological significance.Methods Tissue microarrays of 24 cases with paired squamous cell carcinoma of cervix,cancer adjacent and adjacent normal tissue and 21 cases with squamous cell carcinoma of cervix,6 adjacted carcinoma tissues and 15 lymph hode were investigated immunohistochemically to determine the level of expression of MMP-14 proteins.Results MMP-14 proteins was noticed to be localized in the cytoplasm and at the cell membrane in carcinoma cells and overexpresssion was found in 41 of 45.Overexpression of adjacent carcinoma tissues was found in 3 of 26.Overexpression of lymph hode was found in 2 of 25.Expression of MMP-14 proteins showed a significant differentce between carcinoma and adjacent carcinoma tissues(P=0.000).No significant differentce was found betweet adjacent carcinoma tissue and adjacent normal tissue(P0.05).Expression of MMP-14 protein showed significantly increased in nodal metastasis groups compared with the groups without nodal metastasis(P0.05).Overexpression of MMP-14 proteins was related with mascle invasion and regional lymph hode metastasis and was not associated with histologic grade,advanced T stage and advanced tumor stage.Conclusion These results suggested that Overexpression of MMP-14 proteins was associated with invasion and metastasis and it might play a predictive role in lymph hode metastasis and early tumor progression.
