椎体成形术结合经后路椎弓钉棒系统固定治疗胸腰椎骨转移癌的疗效分析(附19例报告)

The clinical analysis of vertebroplasty combined with posterior fixation with pedicle screw in the treatment of metastastic tumor of thoracolumbar spine(A report of 19 cases)

目的探讨对于已经出现脊髓压迫的胸腰椎转移癌手术治疗的方法和效果。方法对2006-2009年本院收治19例脊柱骨转移癌并出现神经功能受损手术患者,采用后路减压椎弓根系统固定结合骨水泥充填椎体强化治疗,观察术后患者生存质量(包括疼痛、睡眠、食欲)改善程度和神经功能恢复程度。结果全组19例患者中全部脊椎得到稳定,术后患者的疼痛、食欲、睡眠获得显著改善(与术前比较P<0.01),术后三月约78.9%的患者神经功能获得至少Frankel 1级以上恢复。结论椎体成形术结合经后路椎弓钉棒系统固定术式相对简单、安全并可有效改善脊柱转移瘤患者的生活质量。

Objective To assess the surgical treatment methods and effectiveness for the metastases in thoracolumbar spine which have caused spinal cord compression. Methods From 2006 to 2009,19 patients in our hospital with spinal bone metastases and neurological dysfunction had accepted the treatments including posterior decompression and pedicle screw system fixation combined with bone cement filling vertebral intensive treatment and were observed the postoperative quality of life （including pain, sleep, appetite ） and the improvement of neurological recovery. Results 19 patients in all of the spine to be stable, postoperative pain, appetite, sleep was significantly improved （compared with the preoperative P〈0.01）,about 78.9% of patients with neurological function was restored at least Frankel 1 level and above. Conclusion Vertebroplasty with posterior fixation is relatively simple, safe and can effectively improve the life quality of the patients suffered spinal metastases.
