Diagnosis and treatment of Mirizzi's syndrome

目的探讨Mirizzi综合征的诊断及手术治疗对策。方法回顾性分析24例Mirizzi综合征患者的临床资料。结果24例患者分别采取胆囊切除或部分切除,直接瘘口修补或胆囊壁肌瓣修补及Roux-en-Y式肝总管空肠吻合术。24例患者均痊愈出院。结论术前确诊及对Mirizzi综合征的认识程度是术中防止医源性胆道损伤的关键,治疗对策依据术中明确病理分型。

Objective To investigate the diagnosis and surgical management of Mirizzi＇s syndrome.Methods The clinical manifestations of 24 cases with Mirizzi＇s syndrome were analyzed retrospectively.Results Cholecystectomy,partial cholecystectomy,direct closure of fistula or closure with gallbladder wall patch,and Roux-en-Y hepaticoduodenostomy were performed for these 24 cases individually,and all of these cases were cured.Conclusion Correct preoperative diagnosis and understanding to Mirizzi＇s syndrome are keys to prevention for iatrogenic bile duct injury during operation.The treatment should be according to the pathologic type of the Mirizzi＇s syndrome.
