阴道超声引导后穹窿穿刺抽吸盆腔积液临床价值的探讨

A discussion of the clinical value of transvaginal ultrasound-guided culdocentesis in aspiration of pelvic fluid

目的探讨阴道超声引导下后穹窿穿刺抽吸盆腔积液在已婚女性盆腔疾病诊断与治疗中的的价值。方法39例经腹或经阴道超声显示盆腔积液患者,在阴道超声引导下行后穹窿穿刺抽液术,对抽出液体进行化验检查,并对盆腔炎、盆腔积脓患者进行冲洗及药物灌注治疗。结果39例盆腔积液患者在阴道超声引导下行后穹窿穿刺抽液术44次,均一次进针成功,抽液量15-450 ml。无一例出现并发症。结论经阴道超声引导后穹窿穿刺获取积液标本和对感染性盆腔积液抽吸、冲洗和药物灌注治疗为已婚女性盆腔疾病诊断与治疗提供了新途径,对提高治疗效果、缩短治疗时间和减轻患者经济负担具有临床应用价值。

Objective To evaluate the value of transvaginal ultrasound-guided aspiration of pelvic fluid in the diagnosis and treatment of pelvic disease in married women.Methods 39 patients confirmed with pelvic fluid by transabdominal or transvaginal unltrasound examination.Each patient underwent transvaginal ultrasound-guided culdocentesis and the pelvic fluid samples were tested for laboratory results.Flushing and medicinal perfusion were performed in patients with pelvic inflammatory disease or empyema.Results 44 drainages were successfully performed with single catheterizations on 39 patients.Fluid volume varied between 15 ml and 450 ml.No complications were encountered.Conclusion Transvaginal ultrasound-guided drainage of pelvic fluid,flushing and perfusion in patients with inflammatory pelvic fluid is a new method to diagnose and treat female pelvic diseases.It is of significant clinical value for its advantages as improvement of therapeutic efficacy,reduction of therapy duration and the economic burden of the patients.
