胰头癌致十二指肠梗阻经皮内镜下胃造口-空肠置管术并发症处理

ANALYSIS OF COMPLICATIONS OF PEG/PEJ IN PATIENTS OF DUODENUM OBSTRUCTION COMBINING PANCREATIC CANCER

目的总结分析经皮内镜下胃造口(percutaneous endoscopic gastrostomy,PEG)/空肠造口(percutaneous endoscopic jejunostomy,PEJ)并发症的预防。方法回顾分析80例患者行PEG/PEJ术并发症。结果 80例患者均无操作相关死亡,无严重并发症发生。患者发生轻微并发症7例(8.7%):分别为造瘘口渗血1例,造瘘口周围感染1例,在实施肠道营养过程中PEJ管堵塞2例,造瘘口周围肉芽增生1例,PEJ管尖端移位1例,PEG管断裂1例。均通过非手术治疗后治愈。结论合理选择患者、术前必要的检查、术中仔细操作、术后良好护理、重视和加强对患者及家属的健康教育是预防并发症的关键。

Objective To summarize and analyse retrospectively the prevention of complications by percutaneous endoscopic gastrostomy （ PEG ）/percutaneous endoscopic jejunostomy（PEJ）. Methods Analyse the complications in 80 patients who were performed with percutaneous endoscopic gastrostomy （PEG） /percutaneous endoscopic j ej unostomy（PEJ）. ReSults There was no death case and mortal complications associated with the procedure. The incidence rate of slight complications was 8.7%. Including incision errhysis in 1 case, slight inflammation around the incision in 1 case,catheter clogging in 2 cases,hyperplasia in 1 case,catheter moving in 1 case, catherter breaking in 1 case. All the patients were cured through PEG/PEJ, Conclusion The keys of preventing complications are choosing reasonable cases, essential examinations before operation, careful operation, well nursing after operation and reinforcing education of patients and care givers.
