控制性超促排卵对不孕妇女血脂代谢及血清瘦素的影响

The influence of controlled ovarian hyperstimulation to serum lipid metabolism and leptin in infertilily women

摘要 目的:探讨控制性超促排卵(COH)对不孕妇女血脂代谢及血清瘦素的影响。方法:接受体外授精-胚胎移植不孕妇女48例,以自身自然周期为对照,比较自然周期与促排卵周期在不同时间点血清高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、胆固醇(TG)、甘油三酯(TC)、瘦素(LEP)、雌二醇(E2)及孕酮(P)水平的变化,分析COH过程中血脂各因子及LEP的变化规律。结果:自然周期和促排卵周期血脂各指标无明显波动(P>0.05);HDL变化与雌激素水平呈正相关,LDL、TG、TC变化趋势与雌激素水平呈负相关;在自然周期LH出峰日LEP均值水平出现高峰,且明显高于相应自然周期月经第2天的LEP水平(P<0.05);COH周期HCG注射日LEP均值水平亦出现高峰,且明显高于促排卵周期月经第2天的LEP水平(P<0.05);血清LEP与血清E2呈正相关(P<0.05)。结论:短时间应用COH治疗对不孕女性脂代谢无不良影响。 Objective:To explore the effect of controlled ovarian hyperstimulation to serum lipid metabolism and leptin in infertile women and normal women.Method:Forty eight infertile patients who undergone IVF-ET cycles were compared with serum lipid metabolism and leptin in COH cycle to their own natural cycle in different time,which was to analyze the variance of the factors in COH cycle.Results:The serum lipid metabolism level in COH cycle showed rare difference with their natural cycle(P0.05).Changes in HDL levels had a positive correlation with estrogen,and changes in LDL,TG,TC were negatively correlated with estrogen levels.LEP average value level in natural period LH present the peak,and was higher than that of the corresponding natural period menstrual second day obviously(P0.05);COH cyclical HCG injected date LEP average value level also presented the peak,and was higher obviously than the COH cyclical menstrual second day of LEP level(P0.05);serum LEP and serum E2 presented positive correlation(P0.05).Conclusion:There is no adverse effect on the lipid metabolism of applying short time COH treatment to the normal infertile women.
