糖尿病肾病与抵抗素、MMP-2的相关性研究

The relationship study between diabetic nephropathy and resistin and MMP-2

目的 探讨血清抵抗素、超敏C反应蛋白（Hs-CRP）和金属蛋白酶（MMP-2）在2型糖尿病肾病发生发展中的作用.方法 测定正常人30例、2型糖尿病组（单纯糖尿病组30例、微量白蛋白尿组30例、蛋白尿组30例）患者共120例研究对象的血清抵抗素、Hs-CRP和MMP-2水平,并分析其与尿微量白蛋白等指标的关系.结果 2型糖尿病（diabetes mellitus, DM）组血清抵抗素和Hs-CRP水平明显高于正常对照组（P〈0.01）.糖尿病组中微量白蛋白尿组和蛋白尿组两个亚组血清抵抗素及Hs-CRP水平明显高于单纯糖尿病组（P〈0.01）.抵抗素与Hs-CRP和尿微量白蛋白正相关（r=0.67,0.61 P值均〈0.01）.DM组血清MMP-2水平明显低于正常对照组（P〈0.01）.糖尿病组中微量白蛋白尿组和蛋白尿组两个亚组血清MMP-2水平明显低于单纯糖尿病组（P〈0.01） MMP-2和血清抵抗素.Hs-CRP、尿微量白蛋白呈负相关（r=-0.64,-0.60,-0.58 P值均〈0.01）.结论 抵抗素、Hs-CRP、MMP-2与糖尿病肾病发生、发展有关,检测血清其水平可能对早期诊断和评估糖尿病肾病有作用.

Objective To investigate the effect of resistin and supersensitive C reaction protein （hs- CRP） , MMP- 2 on the production and progress in the type 2 diabetic nephropathy. Methods Levels of resistin,hs - CRP and MMP - 2 in 120 subjects were measured; there were 30 eases normal control, 30 cases with T2DM, 30 cases with T2DM accompanied with microalbuminuria and 30 cases with T2DM accom- panied with albuminuria. The relationship between resistin, hs - CRP, MMP - 2, urinary microalbum and other parameters were analyzed. Results Levels of serum resistin, hs- CRP in type 2 diabetic group were significantly higher than those in control group （P〈 0.01）. In dia- betic group, levels of serum resistin and hs - CRP in microalbuminuria and albuminuria groups were signifiectly higher than in patients only with diabetes （P〈 0.01）. There were positive correlation between resistin and Hs - CRP, microalbuminuria （r= 0.67,0.61 ; all P〈 0.01）. Levels of serum MMP - 2 in type 2 diabetic group were significantly lower than those in control group （P 〈 0.01 ）. In diabetic group, levels of serum MMP - 2 in microalbuminuria and albuminuria groups were significantly lower than in patients only with diabetes （ P 〈 0.01 ）. There were negatively correlation between MMP - 2 and resistin, Hs - CRP, microalbuminuria （r = - 0.64, - 0.60, - 0.58 ; all P 〈 0.01 ）. Con- dusions Resistin, hs- CRP and MMP - 2 are correlated with diabetic nephropathy. Levels of serum resistin, hs - CRP and MMP - 2 may have affect on early diagnosis and estimation of diabetic ncphropathy.
