代谢综合征的中医痰证病理与血糖、血胰岛素及胰岛素抵抗的相关性研究

Study on relationship of pathology of phlegm syndrome of TCM with blood sugar,insulin and insulin resistance in metabolic syndrome

目的:探讨代谢综合征的中医痰证病理与血糖、血胰岛素及胰岛素抵抗的关系。方法:采集233例代谢综合征的中医四诊资料,应用"证素辨证"方法计算出痰证积分,同时检测血糖、血胰岛素,计算胰岛素抵抗指数(HOMA-IR)。结果:①空腹血糖、餐后30、60、120、180min的血糖与中医证素痰呈正相关,相关系数分别为0.158(P<0.05)、0.152(P<0.05)、0.102(P<0.05)、0.126(P<0.05)、0.147(P<0.05);②空腹胰岛素、餐后30、60、120、180min的胰岛素与中医证素痰呈正相关,相关系数分别为0.135(P<0.05)、0.245(P<0.01)、0.284(P<0.01)、0.232(P<0.01)、0.153P<0.05);③HOMA-IR与中医证素痰呈正相关,相关系数为0.269(P<0.01)。结论:中医证素痰始终贯穿糖代谢整个过程,痰浊从病理机制上与胰岛素抵抗有一致性。

Objective：To explore the relationships of pathology of phlegm syndrome of TCM with blood sugar,insulin and insulin resistance in metabolic syndrome（MS）.Methods：The information of 233 cases of MS was collected by four diagnostic methods,syndrome differentiation by syndrome element was used to analyze the diagnosis information,and the blood glucose,insulin and HOMA-IR were detected.Results：①Fasting blood glucose and 30 min,60 min,120min,180 min postprandial blood glucose level had positive correlation with phlegm.Correlation coefficient were 0.158（P〈0.05）,0.152（P〈0.05）,0.102（P 0.05）,0.126（P〈0.05）,0.147（P〈0.05）;②Fasting insulin and 30 min,60 min,120min,180 min postprandial insulin level had positive correlation with phlegm.Correlation coefficient were 0.135（P〈0.05）,0.245（P〈0.01）,0.284（P〈0.01）,0.232（P〈0.01）,0.153（P〈0.05）;③HOMA-IR had positive correlation with phlegm.Correlation coefficient was 0.269（P〈0.01）.Conclusions：Phlegm was one of the factors that can influence the whole course of glycometabolism,it had consistency with insulin resistance on pathological mechanism.
