髌骨软化症在MR延迟增强扫描中的表现 被引量:5

The Evaluation of 3D-FS-SPGR Delayed Enhanced MR Scan in Chondromalacia Patella

摘要 目的总结髌骨软化症(chondromalacia patellae,CMP)在MR三维抑脂扰相梯度回波序列(3D-FS-SPGR)延迟增强扫描(dGEMRIC)中的表现。资料与方法分析30例经临床及关节镜确诊的不同时期的CMP患者的3D-FS-SPGR序列dGEMRIC表现。结果30例CMP患者,共35个髌骨,其中I期病灶6个,髌软骨厚薄均匀,内见斑点状、斑片状高信号,不超过全层50%。II期病灶9个,髌软骨内呈斑片状与髌软骨垂直的线状高信号,范围超过全层50%。III期病灶12个,髌软骨局限性变薄,内部见明显高信号强化灶,累及软骨全层,分层现象中断、消失,表面凹凸不平。IV期病灶8个,髌软骨明显变薄,连续性中断,分层现象中断、消失,局部软骨剥脱,软骨下骨裸露。结论MR3D-FS-SPGR序列dGEMRIC能清楚地显示CMP的各期表现。 Objective To summarize imaging features of chondromalacia patella (CMP) on 3D-FS-SPGR dGEMRIC.Materials and Methods The 3D-FS-SPGR dGEMRIC findings of 30 patients with different stage of CMP confirmed by arthroscope were analyzed.Results Among the 30 CMP patients (35 knees in total),6 were diagnosed at stage I with even thickness of knee cartilage,spot and spotty patchy shadows which were less than 50% of the whole layer.There were 9 examples in stage II where the spotty shadows exceeded more than 50% of the whole layer.There were 12 examples in stage III where local knee cartilage became thin and obvious hyperontensity signal involving whole layer,the cartilage surface was irregular.There were 8 examples in stage IV with thin knee cartilage and exposure of local knee cartilage.Conclu-sion 3D-FS-SPGR dGEMRIC can clearly depict CMP in different stages.
