目的:观察海藻酸钠-聚赖氨酸-海藻酸钠(APA)微囊化转鼠前脑啡肽原基因NIH3T3细胞(APA-NIH3T3/rPENK)移植对大鼠神经痛的影响及相关机制.方法:制作SD大鼠坐骨神经慢性压迫模型(CCI),随机分为APA空囊组、NIH3T3/rPENK组和APA-NIH3T3/rPENK组.测定移植后各组CCI术侧热痛阈,大鼠脊髓背角N-甲基-D-天冬氨酸受体2B亚基(NR2B)蛋白的表达情况.结果:移植后NIH3T3/rPENK组和APA-NIH3T3/rPENK组热痛阈明显高于APA空囊组.移植15d后APA-NIH3T3/rPENK组热痛阈显著高于NIH3T3/rPEN组.NIH3T3/rPENK组、 APA-NIH3T3/rPENK组结扎侧背角NR2B蛋白表达低于APA组;APA-NIH3T3/rPENK组背角NR2B蛋白表达低于NIH3T3/rPENK组.结论:NIH3T3/RPENK或APA-NIH3T3/rPENK植入大鼠的蛛网膜下腔可以明显减轻神经痛大鼠的热痛敏感行为,并降低脊髓背角NR2B蛋白的表达.
Objective: To observe the analgesic effect and related mechanisms of intrathecal alginate-polylysine-alginate (APA) microencapsulated NIH3T3/rPENK on neuropathic pain in rats. Methods: CCI-model rats were randomly divided into three groups: APA group, NIH3T3/rPENK group and APA-NIH3T3/rPENK group. The thermal threshold of the operated hindpaws was examinated before and after transplantation in all three groups. NMDAR2B (NR2B) immunopositive neurons in the spinal dorsal horn was detected by immunohistochemistry. Results: The thermal thresholds of the rats in NIH3T3/rPENK group and APA-NIH3T3/rPENK group were mainly higher than that APA group. Fifteen days after transplantation, the thermal thresholds of the rats in APA NIH3T3/rPENK group were higher than those in NIH3T3/ rPENK group. NR2B immunopositive neuron integral optical density in the spinal dorsal horn of the ligation side in APA group was signifieantly higher than that in NIH3T3/rPENK, APA-NIH3T3/rPENK and the non-ligated side of APA group. NR2B immunopositive neuron integral optical density in the spinal dorsal horn in NIH3T3/rPENK group was obviously higher than that in APA-NIH3T3/rPENK and the control. Conclusion: Intrathecal implantation of NIH3T3/rPENK or APA-NIH3T3/rPENK can significantly reduce the pain-sensitive behavior in neuropathic pain rats. It may be associated with the graft releasing of enkephalin and its action on the opioid receptor.
