原发性输卵管癌研究现状与进展

Research status and development of primary fallopian tube carcinoma

原发性输卵管癌发病率低,主要发生于绝经后的妇女,目前病因尚不明确,临床表现为阴道排液、阴道流血、腹痛及盆腔肿块等,影像学上易被误诊为卵巢肿瘤,临床分期类似于卵巢癌,治疗以手术为主,术后辅以化疗和放疗。早期即易发生淋巴结转移,与相同分期的卵巢癌比较有生存优势,诊断时分期、术后残余病灶直径与预后有关。现从病因、临床表现、术前影像学及其它检查手段、组织学分型、分化程度、病理诊断标准、治疗与预后方面进行综述。

Primary fallopian tube carcinoma（PFTC）is a very rare gynecologic malignancy.Tube malignancy mainly affect postmenopausal women in the fifth and sixth decades of their lives.The etiology of this cancer is unknown.The most common symptoms are vaginal discharge,abnormal vaginal bleeding,abdominal pain and a pelvic mass.It＇s frequently misdiagnosed as ovarian tumor.Compared with EOC（epithelial ovarian cancer）,the tumor has the strong tendency for lymphatic spread.Surgery is the treatment of choice for PFTC,with adjuvant chemotherapy and radiotherapy.The patients of PFTC showed survival advantage as compared to the patients of EOC（epithelial ovarian cancer）.Stage of disease at the time of diagnosis and the residual disease are the most important prognostic factors for survival.We summarize the literature on etiology,clinical investigation,preoperative imaging,histologic type,grade,histopathologic diagnostic criteria,treatment and prognosis of PFTC.
