LigaSure血管闭合系统在腹腔镜脾功能亢进脾切除术中的应用(附28例报告)

Application of LigaSure in laparoscopic splenectomy for hypersplenism:with a report of 28 cases

目的:探讨LigaSure闭合系统在脾功能亢进患者行腹腔镜脾切除术中的应用价值。方法:为28例脾功能亢进患者施行完全腹腔镜脾切除术。应用LigaSure离断脾周韧带及部分脾蒂血管。结果:28例手术均获成功。手术时间55～210min,平均120min。术中出血30～220ml,平均115ml(不包括脾血)。22例术后少量腹水。术后12～24h恢复胃肠蠕动,术后24h拔除胃管并进流质饮食,术后住院5～9d,平均6.5d。结论:LigaSure可安全、有效地闭合脾周韧带和大部分血管,减少了术中出血,降低了手术风险。

Objective：To explore the value of LigaSure in laparoscopic splenectomy for hypersplenism.Methods：Twenty-eight patients with hypersplenism were operated by total laparoscopic splenectomy.The perisplenic ligaments and partial splenic pedicle vessels were dissected with LigaSure.Results：Twenty-eight operations were successful.The mean operative time was 120min （range 55-210）,and mean blood loss without spleen blood was 115ml （range 30-220）.22 patients had a little ascites after operation.The time of post-operative gastrointestinal function recovery was 12-24h.The stomach tube was removed and liquid diet was applied 24h after surgery.The mean postoperative hospital stay was 6.5d （range 5-9）.Conclusions：LigaSure is safe and effective in closure of the ligament and most vessels of spleen in laparoscopic splenectomy,and decreases the loss of blood and risk of surgery.
