腹腔镜内容物还纳加完全腹膜外补片植入术治疗嵌顿性腹股沟疝

Laparoscopic content return and total extraperitoneal prosthesis in treatment of incarcerated inguinal hernia

目的:探讨完全腹膜外补片植入术(total extraperitoneal prosthesis,TEP)治疗嵌顿性腹股沟疝的可行性及临床疗效。方法:2005年7月至2009年6月应用腹腔镜探查内容物还纳及TEP治疗31例嵌顿性腹股沟疝患者。结果:31例TEP均获成功,无死亡病例。手术时间127～158min,平均136.4min。术中出血10～20ml。术中均有阴囊气肿,术后自行吸收。并发呼吸道感染2例,术后4d恢复饮食。住院7～11d,平均8.3d。随访4～52个月,平均19.4个月,无复发病例,3例下腹壁轻微感觉异常,无其他并发症发生。结论:TEP治疗腹股沟嵌顿疝是安全可行的。

Objective：To evaluate the feasibility and clinical efficacy of laparoscopic totally extraperitoneal prosthesis （TEP） as treatment of incarcerated inguinal hernia.Methods：From Jul.2005 to Jun.2009,31 patients who suffered inguinal incarcerated hernia were treated by laparoscopic exploration,content return and TEP.Results：All the operations were successful in these 31 patients,and no death occurred.The operative time was 127-158min （mean,136.4min） and estimated intraoperative blood loss was 10-20ml.During operation all the 31 patients had scrotum aerocele,which was spontaneously absorbed after operation.2 patients had respiratory tract infection.All the 31 patients were given food on the 4th day after surgery.Their hospital stay was 7-11d （mean,8.3d）.Follow-up of the 31 cases was 4-52 months （mean,19.4 months）,there was no inguinal hernia recurred.3 patients got light paraesthesia at inferior abdominal wall,there wasn＇t any other complications occurred.Conclusions：Laparoscopic TEP is a relatively safe and effective treatment for inguinal incarcerated hernia.
