脐血胆红素与新生儿病理性黄疸关系的临床研究

Study of the relation between cord blood hemobilirubin and pathological jaundice of newborn

目的探讨脐血胆红素与新生儿病理性黄疸关系。方法采集足月正常新生儿脐静脉血(不含溶血标本),用偶氮咖啡因比色法测定胆红素。生后2～7 d采集股静脉血动态监测血清胆红素。比较不同脐血胆红素范围的新生儿病理性黄疸及治疗的比例。结果500例新生儿中181例(36.20%)发生病理性黄疸,发生病理性黄疸脐血胆红素浓度显著高于未发生病理性黄疸的脐血胆红素浓度(P<0.01)。随着脐血胆红素浓度上升,病理性黄疸发生率增加(P<0.01),需干预治疗病例增多(P<0.01)。结论脐血胆红素水平对预测新生儿病理性黄疸有极大价值。若脐血胆红素检测大于36μmol/L,建议早期干预治疗病理性黄疸的发生。

Objective It is to approach the relation between cord blood hemobilirubin and new born jaundice.Methods 2 mL navel string vein blood of baby were collected after giving birth to（do not contain to dissolve the blood specimen） in the normal newborn,and the hemobilirubin were detected by accidentally nitrogen caffeine ratio color method.The femoral vein bloods were deleted from 2nd day to 7th day after birth.The ration of pathological jaundice of newborn and its treatment were analyzed in different levels of cord blood hemobilirubin.Results 181 cases（36.20%） with pathological jaundice were diagnosed in 500 newborns.The concentration of cord blood hemobilirubin in baby with pathological jaundice was significantly higher than that of the normal newborn（P0.01）.As the concentration of cord blood hemobilirubin was increased,the incidence of pathological jaundice was raised and the cases that needed to intervention treatment was increased.Conclusion The detection of detect the level of cord blood hemobilirubin is very worthy to estimate the occurrence of pathological jaundice of newborn.The early intervention treatment of pathological jaundice is adviced to perform,if the level of cord blood hemobilirubin was over 36 μmol/L.
