高血压合并阻塞性睡眠呼吸暂停综合征的动态血压分析 被引量:6

Analysis of 24-hour ambulatory blood pressure characteristics in Hypertensive patients with obstructive sleep apnea syndrome

摘要 目的分析高血压合并阻塞性睡眠呼吸暂停综合征(obstructive sleep apnea syndrome,OSAS)患者24小时动态血压(ambulatory blood pressure,ABP)的特点。方法 80例高血压患者行多导睡眠仪(polysomnography),行动态血压监测,按监测结果分为单纯高血压病组(essential hypertention,EH)、EH合并轻度OSAS组、EH合并中重度OSAS组。3组进行日间、夜间平均收缩压;日间、夜间平均舒张压;非杓型高血压发生率的对比。结果 EH合并OSAS组?轻度及中重度?夜间平均收缩压、平均舒张压较EH组患者升高,EH合并中重OSAS组日间平均收缩压较EH组患者升高,均有统计学差异。EH合并OSAS组较EH组相比非杓型高血压发生率升高,有统计学差异;EH合并中重度OSAS组升高明显。结论 OSAS与高血压有很强的相关性,EH合并OSAS患者非杓型高血压发生率高于单纯EH组。 Objective To analyze the characteristics of 24-hour ambulatory blood pressure (ABP)in hypertensive patients with obstructive sleep apnca syndrome (OSAS). Methods Eighty hypertensive patients received polysemnography and ABP monitoring. EH group, EH combined with the mild OSAS group and EH combined with the moderate and server OSAS group were divided according to the monitoring results, the daytime, nighttime average systolic, diatolic blood pressure and incidence of non-dipper hypertensive among the three groups were compared. Results The nighttime average systolic blood pressure, average diatolie blood pressure of EH combined with the OSAS group were significant higher than the EH group,the day-time average systolic blood pressure of EH combined with the moderate and server OSAS group was significant higher than the EH group, Non-dipper hypertensive incidence in the EH combined with the OSAS group was higher than the EH group. Conclusion Hypertensive and obstructive sleep apnea syndrome have a strong correlation, the incidence of non-dipper hypertensive in EH combined with the OSAS group is higher than the EH group.
