对一例糖尿病肾病合并高血压和心衰病人的用药分析及药学服务

Drug use and pharmaceutical care for one patient with diabetic nephropathy combined with hypertension and congestive heart failure

目的:探讨临床药师在合理用药中的作用。方法:临床药师通过参与1例糖尿病肾病合并高血压、心衰病人的治疗过程,建议医生选择合适的药物,并根据药物的临床疗效和病情变化及时调整用药方案,提供个体化的药学服务。结果:临床药师为病人提供药学服务,提高了病人治疗依从性和临床治疗效果,减少了药物的不良反应。结论:临床药师在为病人提供个体化药学服务方面具有自身的优势和地位,在合理用药中发挥了重要作用。

Objective： To explore the role of clinical pharmacists in clinical rational drug use.Methods： Through participating in the course of treatment of a patient with diabetic nephropathy（DN） combined with hypertension and congestive heart failure,the clinical pharmacists suggested that doctors should select appropriate drugs for the patient,the regimen should be promptly adjusted according to the drug efficacy and patient＇s conditions,and the patient should be provided with individualized pharmaceutical care.Results： The patient＇s compliance and clinical outcomes were improved,and adverse drug reactions were decreased through pharmaceutical care provided by clinical pharmacists.Conclusion： Clinical pharmacists have its own advantages and status in providing individualized pharmaceutical care for patients.They play an important role in the rational use of drugs.
