纤维支气管镜在大咯血中的应用价值

THE SIGNIFICANCE OF USING FIBERBRONCHOSCOPE IN TREATING MASSIVE HEMOPTYSIS

目的总结纤支镜在大咯血治疗及诊断中的应用价值。方法利用纤支镜对28例大咯血进行检查及治疗。观察大咯血患者的出血部位、血块阻塞情况、向出血部位注入肾上腺素冰生理盐水溶液+凝血酶,并观察止血效果。结果25例明确出血部位,3例室息患者经吸引后呼吸好转,16例经纤支镜治疗后立即止血,7例经纤支镜治疗出血减少,5例患者无效,止血总有效率82.1%。结论急诊纤支镜在大咯血救治中有一定的应用价值,值得基层医院学习使用。

Objective To summarize the applicative value of fiberbronchoscope in the treatment and diagnosis of massive hemoptysis. Methods Fiberbronchoscopy had been used in 28 patients with large hemoptysis for examination and treatment. Observe the bleeding site of massive hemoptysis patients, the condition of obstruction of blood clot, inject into the ice adrenaline saline solution ＋ thrombin, and to observe hemostatic effects. Results In 25 patients find out bleeding site, 3 cases with asphyxia respiration turn better after suction, 16 cases achieved promptly stop bleeding after fiberbronchescopy, 7 cases decrease bleeding, 5 cases no effective, total effective rate of hemostasis is 82.1%. Conclusion Emergency fiberbronchoscopy in the treatment of massive hemoptysis has some value, which is worth learning to be used in the basic hospital.
