球囊PKP治疗20例胸腰椎骨质疏松性压缩骨折 被引量:2

The treatment of percutaneous balloon kyphoplasty for osteoporotic compressed fracture of thoracic and lumbar vertebrae

摘要 目的探讨应用经皮球囊扩张器椎体后凸成形术(PKP)治疗胸腰椎骨质疏松性压缩骨折(OVCF)的临床价值。方法20例29个椎体OVCF患者。在C臂机监测下,经双侧椎弓根穿刺病椎,使用单球囊置入椎体完成扩张,增加病椎高度,填充骨水泥。术后观察疼痛缓解、病椎高度恢复及并发症情况。结果20例患者手术均一次成功,术后24h内疼痛不同程度缓解,无临床并发症。术后病椎前缘和中部高度较术前明显提高。手术前后疼痛及椎体高度比较有统计学意义(P<0.05),1例2处椎旁静脉发现少许骨水泥渗漏,但无临床症状,平均随访6个月未发生严重并发症。结论应用球囊椎体后凸成形术治疗OVCF疗效满意。 Objective To study the clinical effects ofpercutaneous balloon kyphoplasty (PKP) for osteoporotic compressed fracture of thoracic and lumbar vertebrae. Methods 29 vertebrae which had osteoporotic compressed fracture in 20 patients were evaluated. Under the monitor of "G" armed X-ray machine, we did puncture through the pedicles on both sides, and used a single balloon to increase the height of the affected vertebrae so that the bone cement can be filled in. Post-operative observation factors included lumbago alleviation, heights of the vertebral body and complications. Results PKP were performed successfully. The lumbago was alleviated significantly without any complication after operation. The height of the anterior and media vertebral bodies increased obviously in post-operative stage. There were significant differences between pre-operative and post-operative condition in lumbago alleviation as well as the height of vertebral bodies (P〈0.05). 1 patient encountered bone cement leakage into paravertebral vein with no clinical manifestation. The follow-up was 6 months, during which no further vertebral compression occurred. Conclusions The effects of using PKP to treat osteoporotic compressed fracture of thoracic and lumbar vertebrae were satis factory.
