儿童腹型过敏性紫癜的临床特点及诊断

Clinical Features and Diagnosis of Abdominal Anaphylactoid Purpura in Children

目的:探讨儿童腹型过敏性紫癜的临床表现及胃镜检查结果的特点,以提高对腹型过敏性紫癜的认识。方法:回顾分析37例腹型过敏性紫癜患儿的临床资料和胃镜检查结果。结果:37.8%患儿有明确的上呼吸道感染史,腹痛严重而体征轻微,腹痛部位不固定。胃镜检查见胃肠黏膜充血、水肿、糜烂、点片状出血、多发浅溃疡。十二指肠降部黏膜病变最严重,食管黏膜正常,未见胃溃疡。结论:腹型过敏性紫癜可早于皮肤紫癜,早期胃镜检查有助于明确诊断。

Objective：To investigate the clinical characteristics and endoscopy of Henoch-Schonlein Purpura（HSP） in Children,and to increase the awareness rate of HSP. Methods：The clinical characteristics and endoscopy results of 37 cases were analyzed retrospectively. Results：Totally 37.8% of patients with onset before a clear history of upper respiratory tract infection. The symptom the symptom of bellyache was serious,but there was a slight abdomen physical sign.The ache spot was not fixed. Through endoscopy results showed the mucous membrane shows various degrees of hyperaemia,edema,gedaucjed,dotor plateshaped blood,or frequent-bout superficial ulcer. The mucosal lesions became more serious as endoscopy was more closer to the descending duodenurm.The oesophageal mucous was normal and there was no case with gastric ulcer. Conclusions：The children Henoch-Schonlein Purpura（HSP） occur earlier than the skin purpura.Early endososcopic observation can help to diagnose HSP early.
