荧光定量PCR检测大肠癌患者CEA mRNA表达的研究

摘要 目的 通过检测大肠癌病人外周血清中癌胚抗原(CEA),外周血及淋巴结中癌胚抗原信使核糖核酸(CEA mRNA)的相对表达量,建立外周血中定量检测CEA mRNA的方法,并探讨CEA mRNA检测在结直肠癌诊断中的临床意义.方法 采用逆转录-荧光定量PCR技术检测大肠癌病人外周血及淋巴结中CEA mRNA的相对表达量,并用电化学发光法检测外周血中的CEA.结果 50例大肠癌患者手术前外周血CEA mRNA表达阳性率为84%,与正常对照组(20人无1例阳性)比较,差异有统计学意义(P<0.01);CEA 阳性率为22%与CEA mRNA表达阳性率相比两者差异有统计学意义(P<0.01).淋巴结CEA mRNA表达阳性率32.3%,与外周血中CEA mRNA表达阳性率相比两者差异有统计学意义(P<0.01).结论 建立了SYBRGREEN 荧光定量PCR检测大肠癌患者CEA mRNA表达的方法,大肠癌患者手术前外周血CEA mRNA表达阳性率较高,且显著高于外周血中CEA含量以及淋巴结中CEA mRNA表达阳性率,可用于大肠癌患者的术前诊断. Objective To establish a method for quantitative detection of CEA mRNA in the peripheral blood and lymph nodes of patients with cdorectal cancer. Methods Real - time RT - PCR was taken to detect the CEA mRNA comparative expression in lymph node, as well as in the peripheral blood, and ECLIA was taken to detect the value of CEA. Results 50 cases of colorectal, cancer CEA mRNA positive expression in peripheral blood before surgery was 84% , and the normal control group( 10 persons without positive), the difference was statistically significant( P 〈 0.01 ) ; CEA - positive rate of 22% compared with the positive expression rate of CEA mRNA difference was statistically significant( P 〈 0.01 ) ;CEA mRNA lymph node positive rate 32.3% , compared with the positive rate in peripheral blood CEA mRNA difference was statistically significiant (P 〈 0.01 ). Conclusions The method of SYBRGREEN real - time PCR is established for the detection of CEA mRNA expression in patients with eolorectal cancer. The positive rate of CEA mRNA is relatively higher in peripheral blood of preoperative patients than that in lymph nodes, and is relatively higher than the value of CEA. It is useful for the preoperative diagnosis of patients with colorectal cancer.
