角朊细胞和朗格罕细胞在外阴白色病变发病中作用的初步研究

Preliminary research of the effect of keratinocyte and langerhans cell in the vulval white lesion

目的探讨角朊细胞(Keratinocyte,KC)和朗格罕细胞(Langerhans cell,LC)在外阴白色病变发生、发展中的作用,以及其对聚焦超声治疗效果的影响。方法收集42例外阴皮肤标本,其中30例外阴白色病变患者的病损处皮肤,12例正常对照,采用免疫组化法分析标本中KC表达HLA-DR、LC表达HLA-DR和S-100的情况及其与聚焦超声治疗效果之间的关系。结果病变组患者中18例HLA-DR+KC,正常对照组无阳性结果,病变组HLA-DR+LC、S-100+LC明显多于对照组,差异有统计学意义(P<0.01)。HLA-DR+KC患者聚焦超声治疗有效率为88.89%,优于HLA-DR-KC患者的50.00%,差异有统计学意义(P<0.05)。结论KC和LC在外阴白色病变的发生、发展及聚焦超声治疗中起重要作用。

Objective To investigate the effect of KC and LC in the occurrence and development of the vulval white lesion and the efficiency of focus ultrasonic treatment. Methods The expression of HLA-DR in KC and the expression of HLA-DR and S-100 in LC were detected in 42 vulval skin specimens（30 vulval white lesion cases;12 normal cases）by immunohistoehemistry, At the same time, the relationship between the expression and the efficiency of focus ultrasonic treatment was analyzed. Results The expression of HLA-DR in KCs was positive in 18 patients,while negative in all normal cases. The LCs with positive HLA-DR and S- 100 in patients were obviously more than that in normal groups. The efficiency of focus ultrasonic treatment in patients with positive HLA-DR（2 cases were ineffective, 16 cases were effective） was better than that with negative HLA-DR （6 cases were ineffective,6 cases were effective）. Conclusions KC and LC play important roles in the occurrence, development of the vulval white lesion and focus ultrasonic treatment.
