交通伤致髋关节脱位90例临床分析

目的分析交通伤致髋关节脱位的临床特点及治疗效果。方法对90例髋关节脱位治疗进行总结,其中:手法复位33例,手术治疗57例。复位后行骨牵引,牵引6周后行功能锻炼,8周后开始负重行走。结果按Merled’Aubigne髋关节评分系统,复位8周后检查结果:优58例,占64.4%;良19例,占21.1%;中8例,占8%;可3例,占3.3%;差2例,占2.2%.股骨头缺血性坏死2例,占2.2%。诊断为创伤性髋关节炎者21例,占23.3%。结论交通伤致髋关节脱位多合并其他脏器的损伤,在生命体征稳定后需尽早复位。创伤性髋关节炎和关节腔内的骨、软骨碎片密切相关。关节内有碎骨时应手术治疗。治疗时应尽可能保持髋关节的圆滑。

Objective To summarize the traffic dislocation of the hip injury caused by the clinical features and treatment. Methods 90 cases of dislocation of the hip to sum up the treatment,of which：manual reduction of 33 cases,57 cases of surgical treatment. Results Later skeletal traction,traction 6 weeks later exercise,weight-bearing 8 weeks after the walk. According to the results of the Merle d＇Aubigne hip scoring system, the reduction 8 weeks after the test results：excellent in 58 cases,accounting for 64. 4 percent; good in 19 cases, accounting for 21. 1 percent; in 8 patients,accounting for 8% ; in 3 cases Accounting for 3. 3 percent; 2 poor, accounting for 2. 2 percent. Avascular necrosis in 2 cases,accounting for 2. 2 percent. Diagnosed with traumatic arthritis of the hip in 21 cases,accounting for 23. 3 percent. Conclusion Traffic dislocation of the hip injury caused by the merger of other multi-organ damage,in stable vital signs to be reset as soon as possible. Traumatic arthritis of the hip joint cavity and the bone,cartilage fragments are closely related. There should be intra-articu-lar bone surgery. Treatment should be as much as possible to maintain the smooth hip.
