摘要
目的：探讨复杂Pilon骨折的不同手术方法、手术时机及疗效.方法：复杂Pilon骨折32例,根据Reudi-Allowger分型,Ⅱ型20例、Ⅲ型12例,其中10例为开放性骨折,分别采用有限内固定、有限内固定结合外固定支架、解剖型钢板内固定及分期治疗等进行手术治疗.结果：术后随访12～36个月,平均24个月.踝关节功能按Mazur系统评估,其中优20例,良6例,可3例,差3例,优良率81.2%.结论：复杂Pilon骨折根据不同的骨折类型,软组织损伤程度选择不同的手术方式和手术时机,可取得良好的效果.
Objective：To investigate the curative effects of different surgical treatments which were performed at different time points on the complicated Pilon fractures. Methods：According to Ruedi-Allgower classification system, 32 patients with complicated Pilon fractures included 20 cases of type Ⅱ , 12 of type Ⅲ. Amongst them, 10 cases with open fractures accepted the surgical treatments such as limited internal fixation, combination of external and limited internal fixation, internal fixation with anatomic plates, staged strategy,re- spectively. Results ：All the patients had been followed up from 12 to 36 months, average of 24 months. According to Mazur system, the ankle function was excellent in 20 cases, good in 6, fair and poor in each 3. The excellent and good rate was 81.2%. Conclusions：To get the best clinical results, the correct operative approaches and optimal time should be chosen by surgeons who should carefully consider the type of the complicated Pilon fracture and the degree of soft tissue injury.
