社区卫生服务中心部分中医药服务项目的病种成本研究

Disease Cost of Part Traditional Chinese Medicine Service in Community Health Service Center

目的对社区卫生服务中心部分中医药服务项目的病种成本进行研究。方法选取2007年北京市丰台区右安门社区卫生服务中心中医特色诊区的7个病种作为研究对象,先计算病种服务的直接成本,再计算间接成本,求二者之和为总成本;查找2007年中医特色门诊就诊的总人次数;求单次服务平均成本;计算单病种的平均就诊次数;将平均就诊次数和单次服务成本相乘得病种成本。结果中医药服务项目的单次服务成本为29.8元;颈椎退行性变、腰椎退行性变、肩周炎、膝关节退行性变、中风后遗症、面瘫、失眠症的病种成本分别为881.78、630.57、772.71、586.76、932.74、560.84、868.07元。讨论社区中医药服务在收支两条线管理条件下,可以按就诊总人数乘以单次服务成本做年度项目预算,也可以按病种成本乘以病种就诊人数来安排预算;还可以依此来考核社区卫生服务中心的中医药服务。

Objective To investigate the disease cost in service of part traditional Chinese medicine （TCM）. Methods Seven diseases, which were handled with TCM special services at You Amen community health service center in Fantail District of Beijing during 2007, were taken in this study. The direct cost and indirect cost of service for a disease were calculated, and their sum was the total cost. Total number of visitors in TCM special services section during 2007 was collected to find out the average cost of one - time service, then the average visiting number of one disease was calculated; finally the cost of one disease was obtained through multiplication of the average visiting number and the one - time service cost. Results The average cost of one - time TCM service was RMB 29. 8 Yuan. The costs of the degeneration of cervical vertebra, degeneration of lumbar vertebra, per arthritis of shoulder, degeneration of knee joint, sequela of stroke, facial paralysis, and insomnia were respectively RMB881.78, 630.57, 772.71, 586.76, 932.74, 560.84, and 868.07 Yuan. Conclusion With separation of income and expenditure management, the annual budgets of TCM service can be made through multiplication of total number of visitors and one - time service cost, or multiplication of one disease cost and number of visitors for the disease, which can also be used to examine the TCM service in community health service center.
