基层医生对急性有机磷农药中毒的不当治疗分析

目的:探讨基层医生对急性有机磷农药中毒病例中的不当治疗问题及其产生的原因。方法:回顾分析基层医院的50例急性有机磷农药中毒病例的病历资料,总结其中存在的不当问题并分析其产生的原因。结果:基层医生对急性机磷农药中毒的不当治疗主要表现为毒物清除不彻底;胆碱酯酶复能剂应用途径不恰当;阿托品剂量应用或停药时间不当;治疗过程中应用了一些抑制胆碱酯酶活性的药物;呼衰病人呼吸机的使用时机和方法不当。其产生的主要原因是医生对病人病情的错误判断和对某些影响急性有机磷农药中毒治疗疗效的相关因素缺乏了解。结论:正确判断病情是避免不当治疗的关键。

Objective：To explore inappropriate treatment on acute organophosphorus by grassrouts doctors and to analyze its causes. Methods： Retrospective methods were used to analyze medical record data of 50 patients with acute organophosphorus pesticide poisoning in primary hospitals. Points of inappropriate treatment on them were summed up and the causes were analyzed. Results Points of inappropriate treatment performed by grass - roots doctors on pa- tients with acute organophosphorus pesticide poisoning included the following： poison weren＇t clear away completely; giving channel of Pralidoxime iodide was wrong; injection dose of atropine and call off opportunity were wrong;some of drug inhibiting cholinesterase（ChE） activity were be used during therapy acute organophosphorus pesticide poisoning; some of methods on mechanical ventilation to patients with respiratory failure were wrong. Csuses of producting these inappropriate treatment points included：patients＇ condition was evaluated improperly; some physicians were short of comprehend to some factors affecting curative effect of acute organophosphorus pesticide poisoning. Conclusion It is the key factor to raise rate of rescuing acute organophosphorus pesticide poisoning successfully that physician distinguish patients＇ condition correctly.
