摘要
目的探讨重组人生长激素(recombinant human growth hormone,rhGH)在严重烧伤患者中对创面愈合、蛋白质代谢及糖代谢的影响。方法 46例严重烧伤病例随机分成治疗组(n=23)和对照组(n=23),在其他治疗方案均相同的情况下,治疗组伤后第三天每晚21点开始加用rhGH。比较两组患者创面愈合时间,血红蛋白、血浆总蛋白、白蛋白浓度、抗生素应用时间及血糖水平变化。结果治疗组血红蛋白、血浆总蛋白、白蛋白浓度明显高于对照组(P<0.05),抗生素应用时间及创面愈合时间明显缩短(P<0.01),应用rhGH后第4d开始,治疗组空腹血糖水平明显高于对照组(P<0.01)。结论 rhGH能有效促进严重烧伤患者血浆蛋白质的合成,加速烧伤创面的愈合,明显升高血糖的作用。
Objective To explore the effects of recombinant human growth hormone (rhGH) on the healing of wound surface, protein metabolism and the glycometabolism in patients with severe bums. Methods 46 patients with severe burns were randomly and evenly divided into treatment group and control group. All the treatments were same between these two groups, except patients in the treatment group were given rhGH in a dose of 0.3 IU/kg at 9 pm once a day from the third day after the burns. The protein metabolism, healing rat of wound surface and the blood glucose concentration of patients in these two groups were detected. Results The hemoglobin concentration, plasma total protein concentration and plasma albumin concentration in the treatment group were significantly higher than those in the control group (P 〈 0.05). The healing time of wound surfaces and the use of antibiotics in the treatment group were significantly shorter than those in the control group (P 〈0.01 ). Compared with the control group, the fasting blood glucose concentrations were evidently higher in treatment group from the fourth day of beginning use of rhGH. Conclusion rhGH is found to promote protein anabolism, shorten the healing time of wound surfaces and increase the blood glucose concentration in patients with severe burns.
