两种微创方法治疗髌骨骨折的疗效分析

Comparison of Different Minimally Invasive Fixation in the Treatment of Patellar Fracture

目的探讨经皮改良张力带钢丝与改良中空拉力螺钉治疗髌骨骨折的临床疗效。方法髌骨骨折51例,微创应用改良张力带钢丝治疗27例,改良中空拉力螺钉治疗24例,比较两种方法治疗效果及并发症。结果 51例全部随访8~26个月。根据术后膝关节功能恢复情况、内固定失败、疼痛症状和X线片评定疗效。按Shahria-ree评定标准进行综合评价,两组的优良率分别为81.5%和95.8%,疗效差异有显著性意义（P〈0.05）。结论改良中空拉力螺钉微创治疗髌骨骨折,固定可靠,并发症少,疗效满意。

Objective To explore the efficiency of different minimally invasive fixations to fix patellarfracture,with modified AO-tension band fixing or modified cannulated screw.Methods There were 51 patients with patellar fracture,among whom 27 patients were treated with AO-tension band fixing,24 patients were treated with modified cannulated screw.The clinical results and complications were evaluated and the advantages were discussed.Results All of the patients were followed up 8 to 26 months.The curative effects were judged by the functions of the patients＇ Knee joints,the results of X-ray examinations,and complications,the clinical results were evaluated with the Shahriaree score.The excellent and good rates in modified AO-tension band fixing group and modified cannulated screw group were 81.5% and 95.8%.There was significant difference in therapeutic effects（P0.05）.Conclusion Minimally invasive fixations to fix patellar fracture,with modified cannulated screw is more reliable and satisfactory.
