脑室周围-脑室内出血早产儿血小板参数的变化及临床意义 被引量:2

Changes and clinical significance of platelet parameters in premature infants with periventricular-intraventricular hemorrhage

摘要 目的研究脑室周围-脑室内出血(periventricular-intraventricular hemorrhage,PVH-IVH)早产儿血小板参数的变化及临床意义。方法根据头颅彩超检测结果 ,将PVH-IVH早产儿分为轻度PVH-IVH组和重度PVH-IVH组,各55例、16例,并以同期未发生PVH-IVH的早产儿40例作比较,分析比较3组早产儿生后48h内血小板参数的变化。结果血小板计数(PLT)和血小板比积(PCT)重度PVH-IVH组低于轻度PVH-IVH组(P<0.01)和无PVH-IVH组(P<0.01),轻度PVH-IVH组低于无PVH-I VH组(P<0.01),而血小板平均体积(MPV)和血小板分布宽度(PDW)在各组差异不显著(P>0.05)。结论血小板参数的异常与早产儿PVH-IVH密切相关,可能参与了PVH-IVH的病理过程,应重视监测早产儿血小板参数的变化,提高警惕做好PVH-IVH的早期干预。 Objective To explore the changes and clinical significance of platelet parameters in premature infants with periventricular-intraventricular hemorrhage.Methods All subjects were divided into 55 mild PVH-IVH group,16 severe PVH-IVH group and 40 cases without PVH-IVH as control group,which diagnosed in cranial Doppler ultrasonograph.The platelet parameters at their 48 hours of age were analyzed and compared.Results The platelet counts(PLT) and platelet thrombocytocrit(PCT) were significantly lower in the severe PVH-IVH group than in the mild PVH-IVH group(P0.01) and the non-PVH-IVH group(P0.01),and they were significantly lower in the mild PVH-IVH group than in the non-PVH-IVH group(P0.01),while mean platelet volume(MPV) and platelet distribution width(PDW)were similar in all groups(P0.05).Conclusion The changes of platelet parameters are closely related with PVH-IVH,and may be involved in the pathophysiologic processes of PVH-IVH in premature infants.More attention should be paid in monitoring of platelet parameters in premature infants so as to receive the early intervention of PVH-IVH.
