摘要
目的:探讨西宁地区(海拔2260m)10kg以下低体重婴幼儿先天性心脏病的外科治疗的适应证、外科技术及围术期处理。方法:2000年1月—2009年9月,在西宁地区外科治疗10kg以下低体重婴幼儿先天性心脏病47例,体重3.5kg～10kg,年龄1.5个月～2岁,其中中度低温体外循环下手术35例,常温非体外循环手术4例,深低温低流量体外循环下手术8例。结果:住院死亡1例,存活46例,随访2个月～3年,无其他并发症和死亡,均生长发育良好,心功能I级。结论:随着围术期管理、麻醉、体外循环和外科技术的提高,西宁地区10kg以下低体重婴幼儿先天性心脏病外科治疗是安全的,临床效果良好。
Objective：To explore the surgical indications,the management of extra corporeal circulation surgical technology and the peri-operation treatment of congenital heart disease （CHD） in low birth weight less than 10 kg at high attitude（2 260m）.Methods：47children with CHD,aged 1.5 months to 2 years old and weight 3.5-10kg,underwent surgical treatment between January 2000 and September 2009,and of the 47 patients,the median sternotomy with moderate hypothermic cardiopulmonary bypass in 35 cases,with non-cpb （extra corporeal circulation） at room temperature in 4 cases,and with deep hypothermic circulatory arrest in 8 cases.Results：1 case died duration of hospitalization,others were survived. After 2 month to 3 years of follow-up,46 survivors with corrective procedure had no late mortality or complications with NYHA class I of cardiac function.Conclusions：The surgical treatment of CHD in low birth weight（less than 10 kg） infants seems to be safe and good clinical effects with advancement of medical techniques at high altitude.
