摘要
目的通过对603例老年高血压患者的治疗状况的调查,分析其药物应用,血压控制情况。方法采用标准化心血管病流行病学调查方法 ,用问卷调查、查阅健康档案和体格检查相结合的方法。结果 1、调查960名老干部中高血压患者603例,患病率62.81%,其中单纯性收缩期高血压390例(64.68%);高血压病程的中位数为9年,有合并症/并发症541例(89.7%)。2、服用钙拮抗剂(CCB)最多,为75.3%,其余依次为血管紧张素转换酶抑制剂(ACEI)26.2%,血管紧张素Ⅱ受体拮抗剂(ARB)19.4%,β阻滞剂19.4%,利尿剂6.3%,其它药物14.1%。联合用药:1联占52.07%,主要为CCB,2联占36.82%,主要为CCB+ACEI/ARB/β阻滞剂,3联占9.78%,主要为CCB+ACEI/ARB+β阻滞剂。3、血压控制达标占49.9%,控制一般占44.1%,控制差占6.0%。4、按不同联合用药情况、病程长短、合并或并发疾病多少分别分组,各组间血压控制情况均无统计学显著差异(P<0.5)。结论我区老干部高血压的治疗控制情况较好,但有待进一步提高。
Objective To investigate and analyze the treatment,drug application and blood pressure control of 603 senile cases with hypertension.Methods The standard epidemic investigation method for cardiovascular diseases was applied in the investigation as well as questionnaire,archive reviewing and physical examination.Results 1.960 senile carders were involved in the investigation while there were 603 cases with hypertension (including 390 cases with simple systolic hypertension,64.68%),the disease rate was 62.81%;the average time of disease course was 9 years and 541 cases suffered from various complications (89.7%).2.CCB,the most frequent choice,was administrated to 75.3% of all the patients,ACEI to 26.2%,ARB to 19.4%,β-blocker to 19.4%,diuretic to 6.3% and other drugs to 14.1%;out of the combined therapies,52.07% was single therapy,mainly of CCB,36.82% was bigeminy,mainly of CCB+ACEI/ARB/β-blocker and 9.78% was trigeminy,mainly of CCB+ACEI/ARB+β-blocker.3.49.9% of all the cases kept good control of blood pressure,44.1% were just ok but 6.0% failed to control the blood pressure.4.according to the combined therapy,the length of the disease course and kinds of complications,all the cases were divided into several groups and the comparison among groups showed out no statistical difference (P〈0.05).Conclusions Most of the senile senior carders have a better control on blood pressure while more work should be done for further promotion.
