2型糖尿病住院患者焦虑、抑郁状况影响因素分析

Study on Influencing Factors and the Situation of Anxiety,Depression in Type 2 Diabetes Patients in Hospital

目的探讨2型糖尿病住院患者的焦虑、抑郁现状及影响因素,为改善糖尿病患者心理健康状况提供参考依据。方法采用焦虑自评量表(SAS)、抑郁自评量表(SDS)、糖尿病人生存质量特异性量表(DSQL),整群抽取安徽医科大学第一附属医院145例住院糖尿病患者进行调查。结果 2型糖尿病住院患者焦虑和抑郁的发生率分别为42.07%和51.03%。经多因素logistic逐步回归分析结果显示:女性患者(P<0.01)、有并发症(P<0.05)、以半自费(P<0.05)和新农合(P<0.01)为医疗形式的患者更容易出现抑郁情绪;工人(P<0.05)、农民(P<0.01)、病程在10年以上的患者(P<0.01)更容易出现焦虑情绪。结论住院糖尿病患者心理状况较差,存在比较严重的焦虑和抑郁情况。医务工作者和家属应在治疗同时,注意提高糖尿病患者,尤其是具有并发症、病程较长的农民患者的生命质量,对他们进行健康教育和心理关怀。对于已经出现心理疾病的病人进行及时的心理治疗,以全面提高病人康复水平。

Objective To explore the influencing factors and the situation of anxiety,depression in type 2 diabetes,in hospital,and offer the references for improving diabetes psychological health status.Methods 145 diabetic patients were collected by cluster samples from the first affiliated hospital in Anhui Medical University to carry on the investigation by using SAS,SDS,DSQL.Results The incidence of anxiety and depression were 42.07% and 51.03% in hospital in Type 2 diabetic patients.Multivariate logistic stepwise regression analysis found the results as following：Female patients（P〈0.01）,patients who have complication（P〈0.05）,patients half at their own expenses with combing new and agricultural medical form（P〈0.01）are easier to have the depressed mood.Workers（P〈0.05）,peasants（P〈0.01）,patients whose course of disease above 10 years appear anxiety easily.Conclusion Diabetes psychological state is relatively bad in hospital,more seriously anxious and depressed situation exists.Medical worker and relatives should pay attention to improving the life quality of peasant patients who especially have complication and long course of disease during the treatment,showing care for the health education and psychology.Carry on the prompt psychotherapy to the patient who has already presented pshchological disease to improve patients＇recovering level in an all-round way.
