注射用重组人生长激素对缺血型胆道病变模型大鼠的保护作用研究

Protection Effect of Recombinant Human Growth Hormone for Injection on Ischemic-type Biliary Lesion in Rats

摘要 目的:探讨缺血型胆道病变胆管上皮的超微结构变化及外源性生长激素的保护作用,为预防肝移植术后胆道并发症提供实验资料。方法:建立大鼠自体原位肝移植模型,随机分为对照组、缺血型胆道病变(ITBL)组、注射用重组人生长激素(rhGH)组,每组6只。术后当天开始给药,给药7d后获取胆道组织标本。透射电镜观察各组胆管上皮变化,并用图像分析系统对其损伤程度进行定量分析,统计各组胆管上皮细胞面积密度、数密度和微绒毛面积密度等平面计量学参数。结果:与对照组比较,ITBL组胆道完整性受损,胆道上皮微绒毛稀疏,胆管上皮细胞各平面计量学参数均显著降低(P<0.05);与ITBL组比较,rhGH组明显促进胆道上皮增生及微绒毛形态恢复,胆管上皮细胞各平面计量学参数明显升高(P<0.05)。结论:ITBL直接损害胆管上皮细胞,使胆道超微结构破坏,外源性生长激素处理后可明显减轻肝移植模型ITBL,对胆道具有保护作用。 OBJECTIVE: To investigate the changes of ultrastructure of biliary epithelia with ischemic-type biliary lesion (ITBL) and the protection effect of exogenous growth hormone (GH) in order to provide experimental information for prevention of biliary complication after liver transplantation. METHODS: Rats underwent liver transplantation were randomized into control group, ITBL group and recombinant human growth hormone (rhGH) group (n=6, respectively). Each group was given drugs at the same day postoperation. 7 days later, the specimen of bile ducts was collected. The epithelial changes of biliary tract were observed with TEM, and the injury degree of the epithelia was analyzed quantitatively in a specialized image analysis system. The epithelial plane measurement parameters of each group were measured, including the density of area and quantity of epithelia, and the density of area of epithelial microvilli. RESULTS: As compared with control group, in ITBL group the integrity of bile ducts was destroyed, epithelial microvilli became sparse, the epithelial plane measurement parameters were significantly decreased (P0.05). As compared with ITBL group, in rhGH group biliary epithelia proliferation and recovery of epithelial microvilli can be observed and the epithelial plane measurement parameters were significantly increased (P0.05). CONCLUSION: ITBL can directly damnify the epithelia of bile ducts and the ultrastructure of bile ducts. Exogenous GH can significantly alleviate the ITBL after liver transplantation to protect bile ducts.
