肾康注射液联合厄贝沙坦对早期糖尿病肾病患者血清瘦素及脂联素的影响

Influence of Shenkang Injection Combined with Irbesartan on Serum Leptin and Adiponectin of Early Diabetic Nephropathy Patients

目的:观察肾康注射液联合厄贝沙坦对早期糖尿病肾病患者血清中瘦素(leptin)及脂联素(adipone-ctin)的影响。方法:将100例血压正常的早期糖尿病肾病患者随机分为对照组和治疗组,均给予糖尿病教育、饮食控制、适量运动及常规降糖治疗,使空腹血糖<7.0 mmol/L,餐后2 h血糖<10.0 mmol/L;对照组给予降糖药、胰岛素等常规西医治疗,治疗组在对照组治疗的基础上用肾康注射液100 ml加入5%葡萄糖250～300 ml静脉滴注,同时加2～3 U胰岛素加入5%葡萄糖200 ml,每天一次,并加用厄贝沙坦0.15 g,1次/d口服,3周为1疗程;全部病例观察9周,比较两组患者UREA、血清中瘦素、脂联素水平。结果:治疗9周后,治疗组血清瘦素降低、脂联素升高,与治疗前比较P<0.01;且明显优于对照组(P<0.05)。结论:肾康注射液联合厄贝沙坦可减少早期糖尿病肾病患者血清中瘦素,升高脂联素水平,对早期糖尿病肾病具有治疗作用。

Objective：To observe the influence of Shenkang injection combined with irbesartan on urinary albumin excretion rate（UAER）within 24 hours,serum leptin,and adiponectin of patients with early diabetic nephropathy.Methods： One hundred normotensive early diabetic nephropathy patients were randomly divided into control group（group C）,and treatment group（group T）.All of them were given diabetic education,dietary regimen,routine glucose-subduing treatment,and taken suitable movement till their fasting blood glucose（FBG） was less than 7.0 mmol/L and postprandial blood glucose（PBG） less than 10.0 mmol/L.Patients in group C were treated with oral anti-hyperglycemic agents or/and insulin,while those in group T,besides of treatment as control C,were given Shenkang injection 100ml mixed with 5% glucose 250～300 ml plus 2～3 U of insulin intravenously guttaed once a day,plus irbesartan 0.15 g once a day orally.A treatment course was 3 weeks and the experiment lasted 3 courses.The levels of UAER,serum leptin and adiponectin of the 2 groups were detected and compared.Results： UAER,and serum leptin of patients in group T after 9 weeks of treatment were significantly lower than those before and their adiponectin contents were increased（P0.01）.All the improvements were better than those in control group（P0.05）,of which the amelioration of the disease including decrease of UAER and serum leptin and increase of adiponectin was not obvious.Conclusions： Shenkang injection combined with irbesartan can reduce UAER,decrease serum leptin,and increase adiponectin,and has curative effect on diabetic nephropathy in earlier stage.
