脾气虚证与胃粘膜P_(53)基因表达关系的实验研究

Experimental Study on the Relationship between Insufficiency of the Spleen qi and Expression of P 53 Oncogene on Gastric Mucosa.

目的探讨脾气虚证与胃粘膜癌前病变联系的相关癌基因机制。方法胃粘膜癌前病变采用慢性萎缩性胃炎（ＣＡＧ）造模方法；脾气虚证对癌前病变的影响采用脾气虚证与ＣＡＧ同时联合造模方法；胃粘膜病理观察及形态学计量；Ｐ５３基因异常表达采用原位杂交方法。结果脾气虚能加重慢性萎缩性胃炎的程度，与正常对照组比较，ＣＡＧ组与脾气虚型ＣＡＧ组，胃体粘膜固有层厚度明显减少（Ｐ＜０．０１）；胃体腺腺底相对厚度减少（Ｐ＜０．０５）；胃体粘膜核分裂相频数显著增加（Ｐ＜０．０１）。与ＣＡＧ组比较，脾气虚型ＣＡＧ组幽门粘膜核分裂相频数显著增加（Ｐ＜０．０１），贲门固有层淋巴滤泡频数增加（Ｐ＜０．０５），小凹上皮异型改变明显。胃粘膜癌前病变中脾气虚证有Ｐ５３基因的较强表达。结论Ｐ５３基因的异常表达与胃癌前病变辨证中脾气虚证有一定的关系。

e:Objective:To explore the relative oncogene mechanism of insufficiency of the spleen qi and precancerosis of stomach. Methods:Precancerosis of stomach were made on the animal models with chronic atrophic gastritis (CAG). Effect of insufficiency of the spleen qi on precancerosis was observed on the animal models with insufficiency of the speen qi and CAG. Pathological studies were on the gastric mucosa. Expression of P 53 oncogene was detected by in situ hybridization . Results:Insufficiency of the spleen qi can exacerbate the level of CAG. Compared to the control group, both insufficiency of the spleen qi group and CAG group were lower in the thickness of proper mucous membrane of the body of stomach( P <0.01),and the thickness of fundus of gland ( P <0.05). The two groups were higher in the frequency of mucosa karyokinesis in the body of stomach( P <0.01). Compared to the CAG group, insufficiency of the spleen qi group were higher in the frequency of mucosa karyokinesis in the pylorus of stomach( P <0.01),the frequency of lymph follicle of proper mucous membrane in cardia of stomach( P <0.05), and higher in the expression of P 53 oncogene. Conclusion:Insufficiency of the spleen qi has certain relation with the expression of P 53 oncogene in CAG. This can be good to diagnosis and treatment on precancerosis of gastric mucosa based on the theories of TCM.
