腹腔镜模块化胃癌D2根治术患者围手术期护理

Preoperative nursing of laparoscopy-assisted modularized D2 radical gastrectomy for gastric cancer

目的:分析腹腔镜模块化胃癌D2根治术患者围手术期护理,探索更优化的护理对策。方法:回顾性分析35例2007年12月至2009年12月期间接受此手术患者的临床病例资料。结果:全部病例手术均成功,手术平均用时(190.4±30.5)min,平均切口长度(5.9±0.8)cm,平均出血量(120.3±50.6)ml,1例出现切口感染并发症,均治愈。术后平均肛门排气时间及住院时间分别为(2.7±0.5)d、(6.1±0.9)d。结论:腹腔镜模块化胃癌D2根治术是安全可行的,但作为一种新的腹腔镜胃癌手术方式其难度大,术前做好患者的心理、生理护理及术后严密的病情观察、管路护理、并发症的护理、卫生知识宣教是腹腔镜模块化胃癌D2根治术患者围手术期护理的关键。

Objective： To explore a more optimal nursing strategy by analyzing preoperative nursing of laparoscopy-assisted modularized D2 radical gastrectomy for gastric cancer. Methods： The clinical data of 35 patients with advanced gastric cancer received laparoscopy-assisted radical gastrectomy from December 2007 to December 2009 were analyzed retrospectively. Results： Laparoscopy-assistedmodularized D2 radical gastrectomy for gastric cancer was carried out successfully in all cases.The mean operative time was （ 190.4±30.5 ） min, the mean length of incision was （5.9±0.8） cm, the mean blood loss was （120.3±50.6） ml.Only one case suffered from incision infection, all cases were cured.The mean anal exsufftation time and the mean postoperative length of stay were （2.7±0.5） and （6.1±0.9） days respectively. Conclusions： Laparoscopy-assisted modularized D2 radical gastrectomy for gastric cancer is safe.As a new mode of laparoscopy operation for gastric cancer, it is difficult to perform. Before operation, psychological and physiological nursing are needed.After operation, rigorous observation of pathogenetic condition, tube nursing, complication nursing,and health information propaganda are needed.These are keys of preoperative nursing of laparoscopy-assisted modularized D2 radieal gastrectomy for gastric cancer.
