闭合性十二指肠损伤42例临床诊治体会

目的:探讨闭合性十二指肠损伤的临床特点、诊断方法和手术治疗。方法:回顾分析闭合性十二指肠损伤患者42例的临床资料。根据损伤部位、损伤程度,采用单纯十二指肠修补、十二指肠憩室化、胰空肠吻合、胰十二指肠切除术等术式。结果:治愈36例,治愈率85.71%;术后并发症8例,占19.04%。结论:早期正确诊断或及时手术探查,合理选择手术方式,充分引流减压;术后积极抗感染、营养支持及生长抑素的使用可提高十二指肠损伤治愈率。

Purpose： to discuss clinical characteristics, diagnostic procedures and operative treatments of closed duodenum dam- age. Method： to retrospectively analyze clinical data of 42 closed duodenum damage cases. According to position and extent of dam- age, to adopt duodenum neoplasty, duodenum diverticulum, pancreas and jejunum anastomosis, pancreato duodenectomy and etc; Resuit： there were 36 curative cases, a 85. 71% curative rate; after operation, complication occurred to 8 eases, accounting for 19. 04%. Conclusion： early diagnosis or timely operation probe, reasonable operation procedure, sufficient drainage and pressure reduction; infection prevention, nutrition support and usage of somatotropin release in hibiting factor after operations.
