中西结合呼吸康复训练对COPD患者肺功能及生存质量的影响

The Effects of Respiratory Rehabilitation Training on Pulmonary Function and Quality of Life of Patients with COPD

目的:探讨中西结合治疗及呼吸康复训练对慢阻肺(COPD)患者肺功能及生存质量的影响。方法:80例COPD患者随机分为A、B、C、D4组各20例,A组单纯药物治疗;B组配合传统中医气功训练;C组配合现代呼吸肌训练;D组综合前3组的方法综合治疗。训练前及训练后1、3、6及12个月时评定4组患者肺功能指标(FEV1、FEV1%pre,FEV1/FVC%)及COPD生存质量问卷(CRQ)调查。结果:A组训练前及训练后各时间段肺功能指标及CRQ各项评分均无明显改变。B、C、D组训练后1及3个月时各项评定指标与治疗前比较无明显改善;训练后6个月各项指标均明显优于治疗前。与B、C组比较,D组表现更显著(P<0.05)。B组CRQ单因子情感评分高于C组(P<0.05)。结论:现代与传统呼吸康复训练方案均能很好改善COPD患者的肺功能及生存质量,但2种方案结合治疗COPD,则患者的各项评定指标改善更突出。4组治疗时间提示康复训练具有时间依赖性及累积效应,需要长期坚持才能达到理想效果。

Objective：To observe the effects of respiratory rehabilitation training on pulmonary function and quality of life of patients with COPD.Methods：Eighty cases of COPD were randomly divided 4 groups.The patients in group A only received medication,those in group B received modern rehabilitation training,those in group C received traditional Qigong training,and those in group D received comprehensive respiratory rehabilitation training.Pulmonary function and CRQ scale were examined before,and at the first month,third month,6th month and first year after treatment.Results：There were no significant changes in pulmonary function and CRQ scale in group A before and after treatment.There were significant changes in pulmonary function and CRQ scale in groups B,C and D after treatment for 6 months,more significant in group D than in groups B and C（P0.05）.In groups B and D,emotion scale was higher than in group C（P0.05）.Conclusion：Modern rehabilitation training,traditional Qigong training and comprehensive respiratory rehabilitation training programs all can improve pulmonary function and quality of life of patients with COPD,but comprehensive respiratory rehabilitation training program is better than modern rehabilitation training or traditional Qigong training program.Rehabilitation training is time-dependent for the patients with COPD.
