80岁以上高龄老人胆石症56例外科诊治体会

目的探讨高龄老人胆石症的治疗效果。方法回顾性分析2004年1月～2008年12月行开腹胆囊切除术的56例80岁以上高龄老年胆囊结石患者的临床资料。结果全组治愈53例,死亡1例,自动出院2例。术后发生并发症25例。结论高龄胆石症的外科治疗必须针对高龄老人胆道疾病的特点,术前全面检查、营养支持、重要脏器的功能维护、并存病的处理及选择合适的手术时机、麻醉方法与术中监护、并发症的防治等围手术期处理,采取得当的术式,才能减少并发症、提高治愈率。

Objective To explore the therapeutic effect of the oldest old cholelithiasis.Methods 56 cases aged over 80 years old clinical data of patients with gallstones carried on open cholecystectomy in January 2004 to December 2008 were retrospectively analyzed.Results 53cases were cured and 1 died,automatically discharged in 2 cases.Postoperative complications occurred in 25 cases.Conclusion The surgical treatment of cholelithiasis elderly should address the characteristics of biliary tract disease in old age,focusing on a comprehensive examination before surgery,nutritional support,it is important to maintain organ functions,the coexistence of disease treatment and select the appropriate timing of surgery,anesthesia and intraoperative monitoring,concurrent disease prevention and control of perioperative management,surgical properly taken in order to reduce complications and improve cure rates.
