检测胃幽门螺旋杆菌三种染色方法比较

Comparison of Three Staining Methods for Detection of Helicobacter Pylori in Gastric Biopsy Specimens

目的:比较三种检测幽门螺旋杆菌(Hp)的染色方法。方法:收集我科胃镜活检诊断为"慢性胃炎"和"胃溃疡"的蜡块42例,重新切片,分别做HE常规染色,改良Giemsa染色,免疫组化染色。在光镜下观察Hp的显示情况,其阳性率进行χ2检验。结果:三种染色方法的阳性率分别为HE染色:85.7%(36/42),改良Giemsa染色:90.5%(38/42),免疫组化:92.9%(39/42)。χ2检验,免疫组化分别与其他两种染色方法比较,阳性率差异无显著性(P>0.05)。但免疫组化Hp菌体与周围组织对比度最强,最易识别。结论:三种染色方法的阳性率无显著性差异,但采用免疫组化方法Hp菌体最易识别。

Objective： To evaluate three staining methods for detection of Helicobacter pylori in gastric biopsy specimens.Methods： Histological sections from 42 paired gastric biopsies from adult patients previously investigated for chronic gastritis orgastric ulcer were collected,which were stained with HE,modified Giemsa and immunohistochemical（IHC） methods.The results of the three staining methods were compared by χ^2 test.Results： The positive rate was 85.7%（36/42） by the HE stain,90.5%（38/42） by the modified Giemsa stain,and 92.9%（39/42） by the IHC.The positive rate of IHC was similar to the other methods by χ^2 test（P〉0.05）.HP organisms could be easily identified by IHC due to obvious contrast between the bacteria and the surrounding tissues.Conclusion： The positive rate of three staining methods are similar,but IHC is the best choice because of its obvious contrast between the bacteria and the surrounding tissues.
