N-乙酰半胱氨酸和吸入性糖皮质激素对非吸烟慢性阻塞性肺疾病患者炎症和氧化应激的影响 被引量:14

摘要 目的观察N-乙酰半胱氨酸(NAC)和吸入性糖皮质激素(ICS)对非吸烟慢性阻塞性肺疾病(COPD)患者炎症和氧化应激的影响。方法将60例稳定期非吸烟COPD患者随机分为对照组、NAC组和ICS组,后两组分别给予NAC 600 mg,口服,qd,ICS 500μg,吸入,bid。用药12周。用药前后留取患者诱导痰和血清,测定血清谷胱甘肽过氧化物酶(GPx)、血清和诱导痰细胞间黏附分子(ICAM-1)、IL-8浓度。结果纳入患者年龄、性别和肺功能等基础资料差异无统计学意义。经治疗后,两治疗组与对照组比较,血清GPx均升高(P值均<0.05),血清和诱导痰ICAM-1及IL-8均降低(P值均<0.01)。说明两种治疗方法都有一定程度抗炎和抗氧化作用。与治疗前相比,两组患者血清GPx均升高(P值均<0.05),血清ICAM-1、诱导痰IL-8、血清IL-8有所降低(P值均<0.05)。NAC组诱导痰治疗前后ICAM-1无明显变化,而ICS组则降低。相关性分析显示,血清ICAM-1与GPx呈负相关(r=-0.815,P<0.001),与诱导痰IL-8呈正相关(r=0.581,P<0.001)。结论 NAC和ICS对非吸COPD患者均有抗炎和减轻氧化应激的作用,且ICS较NAC作用更强。
