痰热清注射液治疗创伤性急性肺损伤29例临床研究

The protective effect of Tanreqing injection in acute lung injury patients

目的:探讨痰热清在创伤性急性肺损伤的临床肺保护作用。方法:选择59例创伤性急性肺损伤患者按照性别、年龄、急性生理学与慢性健康状况评分系统(APACHEⅡ )的评分标准基本相同的原则,随机分为痰热清组(29例)和常规治疗组(30例),比较治疗7d前后两组患者动脉血气、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)、SIRS评分的变化;比较两组患者ARDS的发病率和病死率。结果:治疗7d后痰热清组较常规治疗组患者的动脉血气、SIRS评分较治疗前有明显改善(P<0.05),痰热清组较常规治疗组血清IL-6、IL-8、TNF-α水平治疗后较治疗前有降低(P<0.05);ARDS的发病率和病死率痰热清组较常规治疗组无显著性差异(P>0.05)。结论:痰热清能降低血清TNF-α、IL-6、IL-8炎性因子的水平;能缓解炎性反应综合征的进展;能改善通气,缓解呼吸窘迫。

Objective：To explore the protective effect of Tan Re Qing injection on acute lung injury（ALI）patients.Methods：59 of ALI patients were enrolled in our study subject and randomly divided into two groups according to the principle of basi-identical in sex,age and APACHEⅡ score.One group including 30 patients was served as general treatment group,and the other including 29 patients was served as Tan Re Qing injection treatment group in base of general treatment.Indices of arterial blood gas analysis were compared between general group and Tan Re Qing treatment after 7 days＇ treatment.Indices of IL-6,IL-8,TNF-α were compared between general group and Tan Re Qing treatment after 7 days＇ treatment.To compare the change of SIRS score.To compare the difference of Morbidity of ARDS.Results：After 7 days＇ treatment,the degree of improvement of indices of arterial blood gas analysis and SIRS score in Tan Re Qing group was more than that in general group（P0.05）;and the degree of lower of indices of IL-6、IL-8、TNF-α in Tan Re Qing group was more than that in general group（P0.05）;Morbidity of ARDS have no difference at statistics between general group and Tan Re Qing treatment group（P0.05）.Conclusion：Tanreqing injection can down the blood-serum level of inflammatory factor as TNF-α,IL-6 and IL-8;can relieve the progress of SIRS and can ameliorate gas exchange in ALI,and slower degree of respiratory distress.
