摘要 目的研究HLA-DQ基因与成人发病的IDDM和成人缓慢进展型IDDM的相关性。方法利用PCR/SSP技术检测了30倒成人发病的IDDM患者、52例成人缓慢进展型IDDM患者和50例正常人的HLA-DQ基因频率。结果①HLA-DQA1*0301、0501及DQB1*0201等位基因与IDDM和缓慢IDDM呈显著正相关;DQB1*0301与缓慢IDDM呈显著正相关,而DQB1*0302与IDDM呈显著正相关。②DQA1*0104、0201及DQB1*0601等位基因与缓慢IDDM呈显著负相关。③HLA-DQα链52-Arg(+)纯合子基因型和DQβ链57-Asp(-)纯合子基因型与IDDM和缓慢IDDM均呈显著正相关,而HLA-DQα链52-Arg(-)纯合子基因型和DQβ链57-Asp(+)纯合子基因型与IDDM和缓慢IDDM均呈显著负相关。结论提示IDDM和缓慢IDDM虽然均为自身免疫性糖尿病,但其HLA表型并不完全相同,不同的HLA表型可能是决定患者起病方式及病情发展的不同因素。 Objective To probe into the association of HLA-DQ gene with IDDM and slowly progressive IDDM(SPIDDM) in adults. Methods HLA-DQ alleles in IDDM patients (n = 30), SPIDDM patients (n = 52) and healthycontrols (n =50) were studied by PCR/SSP techniques. Results ①The frequencies of HLA-DQA1 * 0301,0501 andDQB1 * 0201 were significantly higher in the IDDM and SPIDDM patients; the frequency of DQB1* 0301 wassignificantly higher in SPIDDM patients;the frequency of DQB1 * 0302 was significantly higher in IDDM patients. ②The frequencies of HLA-DQA1 * 0104, 0201 and DQB1 * 0601 were significantly lower in SPIDDM patients. ③Thefrequencies of HLA-DQQ-52-Arg homozygote and DQβ-57-non-Asp homozygote were significantly higher in both IDDMand SPIDDM. But the frequencies of HLA-DQα-52-non-Arg homozygote and DQβ-57-Asp homozygote were significantlylower in both IDDM and SPIDDM. Conclusion. HLA-DQ gene contributes susceptibility and resistance to IDDM andSPIDDM. Compared with IDDM, SPIDDM may be related to different susceptible and resistant gene.
