头颈部恶性肿瘤转移淋巴结用超声引导穿刺活检的临床应用评价

Diagnostic value of ultrasound guided puncture biopsy in patients with cervical lymphatic metastasis from head and neck neoplasms

利用自动弹射活检装置在超声引导下对２３例头颈部恶性肿瘤颈部淋巴结穿刺活检。以颈淋巴结清扫术或颈淋巴结探查术后对淋巴结检查的病理结果为标准，比较超声引导穿刺活检与临床触诊对淋巴结转移的诊断准确性，超声引导穿刺活检诊断符合率８６．９６％，较临床触诊６９．５７％高，假阴性率低，无假阳性，两种方法对诊断淋巴结转移的准确指数具有显著性差异。提示超声引导穿刺活检可以提高头颈部恶性肿瘤淋巴结转移诊断准确率，对于确定术式和预后估计显示了其较强的优势。因此头颈部恶性肿瘤应常规用超声检查颈部淋巴结。

Abstract This article is designed to study the diagnostic value of ultrasound guided puncture biopsy in patients with cervical lymphatic metastasis from head and neck neoplasms.A fixator node by Aloka Corp.and automatic puncture biopsy equipment by Bard Corp.was used to gain the tissue of neck lymph nodes.The results showed that the diagnostic accordance rate of cervical lymphatic metastasis using ultrasound guided puncture biopsy was 86 96%,higher than that of palpation.We suggest that ultrasound guided puncture biopsy should be routinelly used in the diagnosis in the patients with head and neck neoplasms who were suspected greatly to be suffer from lymphatic metastasis.
