不同胎龄早产儿血清蛋白生化指标临床研究 被引量:4

摘要 目的通过测定不同胎龄早产儿血清白蛋白、前白蛋白生化指标的变化,探讨其作为评价早产儿成熟度、营养状况及营养支持的临床意义。方法早产儿组30例为入院时生后24h内的早产儿,入院后给予防治早产儿并发症、尽早给予胃肠道喂养和部分或完全静脉营养,补充水-能量-氨基酸等各种营养成分。分别于出生后24h内及治疗后2周采集股静脉血2ml测定血清白蛋白(albumin,ALB)、前白蛋白(prealbumin,PA)水平。结果 1)早产儿组治疗前血清ALB、PA水平均低于治疗后,两者之间比较存在显著性差异(P<0.01)。2)大于等于28周小于32周的早产儿血清ALB和PA水平均低于大于等于32周小于37周的早产儿水平,两组之间比较存在显著性差异(P<0.01)。结论 1)PA可做为临床评价早产儿成熟度及营养状况的敏感指标,在早产儿出现重度黄疸、水肿时,ALB为临床注射白蛋白提供治疗依据。2)早产儿血清ALB、PA对于早产儿临床营养支持等方面有一定指导作用。 Objective To explore clinical significance of biochemical indexes for serum albumin, prealhumin of premature infants in different gestational ages as the evaluation of the maturity of premature infants, nutritional status and nutritional support, by measuring biochemical changes in serum protein in premature infants in different gestational ages. Methods Preterm infants group, 30 cases of infants admitted to hospital within 24 hours after birth when the preterm infants, preterm infants after admission given to prevention and treatment of complications of enterable feeding as soon as possible and to give partial or total parental nutrition, add water--energy--amino acids and other nutrients. Respectively, within 24 hours after birth and collected 2 weeks after treatment, the femoral vein 2 ml serum albumin (albumin, ALB), prealbumin (pre albumin, PA) level. Results (1)Before treatment Serum ALB, PA levels of premature infants were lower than that after treatment. The result was different significantly (P 〈 0.01). Serum Alb and PA levels of premature infants ≥28〈32 weeks were lower than those 32〈37 weeks. The level between the two groups of premature infants was significantly different (P 0.01). Conclusions OPA can be used as clinical evaluation of the maturity of preterm infants and sensi- tive indicator of nutritional status in preterm infants had severe jaundice, edema, ALB of providing treatment based on clinical injection of albumin. (2)Serum ALB, PA of premature infants play the role of some guidance to clinical nutritional support.
