摘要
目的:提高对皮肌炎伴发急性肺间质病变临床表现和治疗的认识。方法:总结3例皮肌炎伴发急性肺间质病变的临床表现及治疗经过。结果:皮肌炎伴发急性肺间质病变起病迅速,多为感染诱发,临床表现为胸闷、紫绀,严重低氧血症和呼吸衰竭,高分辨肺CT有很大的诊断价值,早期大剂量激素联合环磷酰胺,合并感染者积极抗感染,综合治疗可收到良好的效果。结论:皮肌炎继发急性肺间质病变是临床急危症,临床治疗过程中应制定个体化方案。
Objective：To describe the clinical features and therapy of dermatomyositis with acute interstitial lung disease.Methods：The clinical features and therapy of dermatomyositis with acute interstitial lung disease were reviewed.Results：Dermatomyositis with acute interstitial lung disease was a rapid process.Mostly induced by infection.The symptoms were hard on breathing,cyanosis,dyspnea,severe hypoxemia and respiratory failure.High resolution CT had great diagnostic value.High-dose steroid and cyclophosphamide,combined with active anti-infective,combined treatment can be proved to be effective.Conclusion：Dermatomyositis with acute interstitial lung disease was acute risk of clinical disease,we should make a individual programs.
