微创神经减压术治疗糖尿病性周围神经病临床研究 被引量:2

摘要 目的探讨微创神经减压术对糖尿病周围神经病的治疗效果。方法对20例糖尿病周围神经病患肢进行微创神经减压术,观察治疗前后患者症状的改善程度以及患肢正中神经、腓总神经的运动神经传导速度,正中神经、腓浅神经的感觉神经传导速度的改变程度,并与同期住院病人保守治疗组进行对照。结果手术组临床治愈11例,显效4例,有效3例,无效2例,总有效率为90%;对照组显效10例,有效5例,无效5例,总有效率为75%,手术组的疗效显著高于对照组(χ2=20.01,P<0.01)。手术组治疗后神经传导速度与治疗前比较明显加快(P<0.05)。对照组治疗前后神经传导速度变化无显著性差异(P>0.05)。结论微创外周神经减压术对糖尿病周围神经病变治疗有较好的效果。优于单纯保守治疗,且创伤小,疗效满意,值得推广应用。 Objective To observe the curative effects of minimally invasive nerve decompression surgery on cuabetic peripheral neuropathy. Methods 20 limbs of diabetic peripheral neuropathy received minimally invasive nerve decompression treatmen. The conduction velocities of median nerve, common peroneal nerve, motor nerve and superficial peroneal sensory nerve in the treatment group were compared with those of the patients receiving conservative medical management before and after the treatment. Results In the treatment group, 11 patients recovered completely, 4 got significant curative effect, 3 got some curative effects, and the other 2 got no ef- fects. The total effective rate was 90 % (~ = 20.01, P 〈 0.01). After treatment, the conduction velocities of median nerve, common peroneal nerve, motor nerve and superficial peroneal nerve sensory nerve increased significantly faster. No significant changes in conduction velocity occurred in the control group( P 〉0.05). Conclusion Minimally invasive peripheral nerve decompression can producesatisfactory curative effects.
